You’ll receive:

• A Liability Waiver

Both must be completed and returned at least 48 hours before your consultation.

Your intake form is an essential part of the process. Please take your time and include as much detail as possible — the more insight we have, the more precise and effective your remedies can be.

The form will ask for details such as:

• Personal information and contact details

• Your main reason for attending and health goals

• Current treatments, medications, and supplements

• Lifestyle details such as diet, sleep, energy, and exercise

• Emotional well-being, fears, traumas, and life events

• Family medical history

• For women: menstrual and reproductive health information
